SMS Terms & Conditions
1. Program Description
By opting in to receive text messages from Diane Brzezinski, D.O., P.A. (“we,” “us,” “our”), you agree to receive SMS/MMS messages related to appointment scheduling, appointment reminders and confirmations, prescription and lab notifications, billing and account notices, customer care follow-up, and occasional practice updates or promotional offers about our services.
2. Consent
Consent is obtained through our website forms, patient intake and registration paperwork, in-office or verbal request, or by texting our published number. Consent to receive text messages is not a condition of purchase or of receiving any medical service or treatment. Message and data rates may apply.
3. Message Frequency
Message frequency varies based on your interaction with our office. You may receive up to 4 messages per month, plus transactional messages such as appointment reminders and replies to your inquiries.
4. Costs
Message and data rates may apply. These charges come from your mobile carrier and are your responsibility. We do not charge a fee for the SMS program.
5. Opting Out (STOP)
You may cancel at any time by replying STOP to any message from us. After you send STOP, we will send one confirmation message and no further texts will be sent unless you opt in again. You may also opt out by calling our office at (239) 261-9990 or emailing [email protected].

7. Carriers and Delivery
Carriers are not liable for delayed or undelivered messages. Delivery is subject to effective transmission by your mobile carrier and is not guaranteed. Supported carriers include AT&T, Verizon Wireless, T-Mobile, Sprint, U.S. Cellular, and others; carrier participation may change without notice.
8. Eligibility
You must be at least 18 years of age and the account holder or authorized user of the mobile number provided to enroll in this program.
9. Privacy and Data Sharing
We respect your privacy and will only use the information you provide to deliver the messages described above and to service your account. No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Information may be shared with subcontractors and service providers strictly to support the delivery of these messages (for example, our messaging platform), and those parties may not use it for any other purpose. Text messaging originator opt-in data and consent are not shared with any third parties. See our Privacy Policy for full details.
10. Protected Health Information
Standard text messaging is not a fully secure or encrypted form of communication. By opting in, you acknowledge that messages may contain limited health-related information (such as appointment or prescription details) and you accept the associated risk. Please do not use SMS for medical emergencies — call 911 or go to the nearest emergency room. Do not send sensitive clinical questions by text; contact the office directly instead.
11. Changes to These Terms
We may modify or terminate this messaging program or these terms at any time. Changes are effective upon posting to this page.
